粉防己碱对豚鼠左心室前负荷增加所致电生理改变的影响

摘要(Abstract):

目的 :探讨粉防己碱对左心室前负荷增加时心脏电生理变化的影响及其机制。方法 :采用豚鼠离体工作心模型 ,通过调节左房灌注压改变左室前负荷 ,观察粉防己碱和维拉帕米对前负荷增加前后心肌单相动作电位时程 (MAPD90 )、振幅 (MAPA)、有效不应期 (ERP)和室颤阈 (VFT)的影响。结果 :前负荷增加引起MAPD90 和ERP缩短 ,MAPA和VFT下降 (P <0 .0 1) ;粉防己碱可有效抑制前负荷增加引起的心脏电生理变化 (P <0 .0 1) ;而维拉帕米除对此无明显影响 (P >0 .0 5)。结论 :粉防己碱可能通过阻滞牵张激活性离子通道而抑制前负荷增加引起的心肌电生理特性改变。
Objective: To investigate the changes of g ui nea pig heart electrophysiological properties caused by increasing left ventricu lar preload, and to assess the effects of tetradrine on these changes. Method: Working model preparation of guinea pig hearts in vitro was used, and the preload of left ventricle was increased by adjusting the prefusio n pressure of left atria. The changes of heart electrophysiologic parameters inc luding monophasic action potential duration(MAPD90), monophasic action potential amplitude(MAPA), effective refractory period(ERP) and ventricular fibrillation threshold(VFT) were observed before and after altering the preload of left ventr icle, and compared in the absence and presence of tetradrine,streptomycin or ve rapamil. Result: The rising of left ventricular preload le d to shortening of MAPD90, ERP, and to descent of MAPA, VFT(all P<0.01). Bot h Tetradrine and streptomycin inhibited these changes of heart electrophysiologi c parameters caused by elevation of left ventricular afterload (all P<0.01) . In contrast, verapamil had no effects on the preload-related electrophysiolo gical changes(all P>0.05). Conclusion: Electrophysiolo gic changes caused by increasing left ventricular preload may be inhibited by te trandrine, through inhibition of stretch-activated ion channels.
